How old would your grandchild have to be before you would suprise her and her parents with a family cruise?

As a Nana, IMHO, I would say 4 years old at least. They may enjoy being away from Mom & Dad in the Camp Carnival program, enjoy going on tours to beaches, etc. Eat & drink normal table food and enjoy the music on the pool deck, etc. We hope to do a family cruise one of these days as soon as my son-in-law gets discharged from his mobility with the Air Force. The little guy is just 4 and is a momma's boy. We took the 3 grand kids on a weeks vacation this past summer (sisters are 11 & 8) and you really had to keep everyone busy. If they didn't have plans to go somewhere (Pocono Mountains) they became crabby. The 3 year old at that time was actually the best one of the 3 children. Carnival has great programs for the children.
Bon Voyage

I surprised my 12 year old grandchild with a cruise for Christmas and we just returned 12/15 and she had the time of her life. Made many friends on the cruise. They all exchanged e-mail addresses and will become electronic pen-pals.

IMHO, I believe any travel that may require patience and flexibiltiy should wait until a chld is at least 6 years of age. And then, the routine needs to be modifed to meet the child's needs. We took our first cruise when my oldest was 10 and youngest 7; I believe that at that age the boys behaviours were ready for excursions of the cruise type. That is , not land based day trips and outings. They were old enough to explore, enjoy and remember their experiences. From this trip, they established a desire for cruising and long distance travel. Of course, family vacations were geared around their interests. My husband and I will cherise the memories, however, we have now graduated to empty nesters, and enjoy celebrating this new chapter in our lives.
